Voluntary Disability Disclosure (VDD) Form
§ 30.11 Invitation to self-identify as an individual with a disability.
(a) Pre-offer. Individuals applying for the Apprenticeship Program.
As a Sponsor of the Apprenticeship Program, we invite you to complete a Voluntary Disability Disclosure form.
(b) Post offer. Eligible Applicants with a Conditional Offer of Apprenticeship.
As a Sponsor of the Apprenticeship Program, we invite you to complete a Voluntary Disability Disclosure form.
(c) Apprentices. Current Indentured Apprentices.
As a Sponsor of the Apprenticeship Program, we invite you to complete a Voluntary Disability Disclosure form or to update your form if your status has changed. You will be reminded yearly that you may voluntarily update your disability status.
(d) Voluntary self-identification for Applicants and Apprentices. As a Sponsor of the Apprenticeship Program, we do not compel or coerce any individual to self-identify as an individual with a disability.
Should you choose to complete the Voluntary Disability Disclosure form, you may print the form using this link and email to training@hetf1186.org. All responses are kept confidential in accordance with § 30.12(e).
